Services We Offer
Bourbon Heights, Inc. is a non-profit facility that offers a multitude of services. These services include Skilled Nursing Facility Services, Intermediate Care, Personal Care, Adult Day Health Care Center, Outpatient Rehabilitation Center and Independent Living Apartments. It is our goal to serve the residents of Bourbon and surrounding counties in short or long term stays and to provide quality services to all.
Bourbon Heights, Inc. partners with all Bourbon County physicians to provide physician services for its residents. This allows residents to keep their existing physician while transitioning into our facility or back to home after a short term stay. Dr. Nathan Moore is our Medical Director, he is responsible for overseeing all patient care and providing input on providing quality care to all residents.
Our on-site country chapel is a quiet place to reflect or a great place to listen to church services on Tuesday mornings. There are also several gospel groups that love the acoustical aspects of the chapel and share their joy for music and God. The chapel has hosted several weddings, funerals, and our annual memorial service.
The Joseph Allen Adult Day Health Center is a day program for aged or disabled individuals age 21 or above. This is a great resource if you or your loved one are not safe to stay home alone while caregivers work during the day. The center is open 7:30 a.m. to 4:00 p.m. and is available Monday through Friday. All person centered services are under the direction of medical staff, including Registered Nurse, Licensed Practical Nurse, and State Registered Nursing Assistants. The center offers socialization, activities, nutritional meals and snacks, and medical services if directed by your physician.
Bourbon Heights, Inc. offers both inpatient and outpatient rehabilitation services. Inpatient stays can range from a few days up to 100, dependent upon your insurance benefits and personal improvement and needs. We partner with therapists from Functional Pathways, who are experts in working with different injuries and strengthening exercises. Whether you need physical, occupational, or speech therapy, we have the therapist to meet your needs.

As Administrator, Charlotte Roberts is responsible for the daily operations of the facility as directed by the Board of Directors, ensuring quality resident care and fulfilling the mission of Bourbon Heights, Inc.
Charlotte obtained her Associates in Accounting from Maysville Community College and her Bachelor’s Degree in Health Care Administration from Midway College. Working at Bourbon Heights, Inc. since 1995, Charlotte started at the ground level and worked her way to Administrator in 2014.
As a single mom, Charlotte enjoys spending time with her daughters, Katy and Kristin, reading and swimming in her spare time.
Deanna Eads, R.N.
Director of Nursing
As Director of Nursing, Deanna Eads oversees the nursing department, which is the largest department and is the heart of resident care. Deanna can often be found interacting and helping the residents and nursing employees throughout the day and ensuring they have the knowledge, skill, and compassion to carry out their responsibilities With 109 residents, the nursing department never stops, often receiving calls in the middle of the night and on weekends, Deanna is always available for the residents and employees.
Deanna began her career at Bourbon Heights in February 2003 as a Unit Coordinator and assumed the responsibility of Director of Nursing in July 2003. Deanna enjoys spending time with her daughters and stepsons, along with her 14 grandchildren. She also enjoys reading and sewing in her spare time.
Admissions/Business Office Manager
As Admissions and Business Office Manager, Julie is usually the first person that families meet. Julie shares her compassion from being a State Registered Nursing Assistant since 2009 to help guide residents and families through the admission process. After completing her Bachelor’s Degree in Healthcare Administration, Julie transitioned to the office where she uses her knowledge to help in all aspects of the office, including accounts receivables, accounts payable, payroll, employee benefits, resident trust, and financial statements.
Julie has 2 beautiful children and lives in Paris with her husband. She enjoys doing activities with her children, gardening, and kayaking with her family.
